JOB SUMMARY

The Home and Community Care Registered Nurse contributes to the assessment, planning, implementation, and evaluation of client care. Depending upon the needs and complexity of clients, the RN may assume an independent, interdependent and (often) overlapping role with the healthcare team. The RN works in a collaborative role with other Stoney Trail Wellness Center service providers as well as with local community organizations and other external partners-in-care to provide professional nursing care for assigned clients and their families in their own homes, in alternate care settings and/or in the community.

DUTIES AND RESPONSIBILITIES

  • Work within a defined scope of practice standards developed by the College of Registered Nurses of Alberta (CARNA)
  • Conducts a holistic assessment of the client’s health and wellness needs, family supports and the home environment and consults/collaborates with the HCC Case Manager/RN/HCA
  • Collaborates with the HCC Case Manager in developing, implementing, updating and timely documenting of a comprehensive, client/family-centred care plan
  • Working from a trauma-informed care approach, the RN helps to promote the physical, intellectual, emotional, psychosocial and spiritual well-being of clients/families
  • Conduct regular visits to clients’ homes and alternate care settings and performing direct nursing care within the RN scope of practice
  • Participate in case of conferences (as required), discharge planning and referrals to appropriate health care providers, hospitals and/or community agencies and services are also part of the HCC RN role.
  • Enhancing the self-management capacity of clients, family members and caregivers to encourage optimal independence and self-care;
  • Teaching and supporting other departments and Health Care Aides (HCAs);
  • Providing guidance, field experience and supervision of RN/LPN students when required.
  • Performing administrative and clerical functions related to his/her roles.
  • Implementing and updating policies, protocols, and procedures relevant to your role and program goals.
  • Data collection and analysis to monitor program services for improvement.
  • Community engagement, prevention, and education for clients.
  • Other duties as required by Supervisor

EDUCATION, SKILLS, AND QUALIFICATIONS:

  • A graduate of a CARNA-approved Registered Nurse Degree Program;
  • Registration in good standing with the College of Registered Nurses of Alberta (CARNA)
    • Valid First Aid and CPR
    • Current Transportation of Dangerous Goods Certification;
    • Current WHMIS Certification.
    • CARNA-approved Advanced Practice Foot Care Certification an asset
    • Immunization Certification and asset
    • Phlebotomy Certification an asset
  • A minimum of one (1) year experience in HCC or a related clinical field;
  • Experience with or interest in Indigenous culture;
  • Possess excellent communication (oral and written)

EMPLOYMENT CONDITIONS:

  • Clear Criminal record check and vulnerable sector
  • Class 5 Advanced Drivers License and Clean Driving Record
