Job Summary:
As a Transition to Adulthood Program (TAP) / Post-Majority Support Services (PMSS) Caseworker, you will support voluntary adults over the age of 18 who were previously in care. This role requires a strong understanding of colonialism, intergenerational trauma, and the lasting impact of the residential school system on Indigenous communities. It is essential to recognize how these ongoing effects influence the experiences of young adults transitioning out of care, some of whom may also be parents.
Tasks, Duties and Responsibilities:
- Complete review of new client files, flagging education/employment/life skills ad/or any other special considerations.
- Review of current transition plan, client goals and areas of connection
- Identify priority client needs (housing, program application, assessments, etc.)
- Participate in case conferences with casework practitioners, youth, and other supports
- Maintain a functional working relationship with all members of the provincial TAP team, and have an understanding of positional roles.
- Maintain one- on-one relationship with all clients on assigned caseload, and celebrate the important life events as they pertain to their culture/ heritage/ spirituality.
- Document all client contact and relevant case notes
- Consult with client on needed life skills, and actively arrange and monitor their acquisition.
- Establish/ maintain and foster ongoing cultural connections
- Arrange / negotiate the funding for each client based on established standardized rates
- Be an advocate for TAP/PMSS clients, both in the community and within the ministry
- Collaborate with the resources available in the communities of Bighorn, Eden Valley and Mini-Thni
- Other duties as assigned by Supervisor

- CS RESP Program,
- PDD, AISH, Alberta Supports
- The Office of the Public Guardian,
- Justice and Solicitor General
- AHS
- Addiction supports
- Office of the Child and Youth Advocate
- Post Adoption Registry Support
- Provincial TAP Policy
- Federal PMSS Policy
- CYFEA & Act 92
Education, Skills & Qualifications:
- Degree in a related field, equivalencies will be considered
- Experience in Youth work, working with young children
- Must have effective written and verbal communication skills; ability to speak Stoney Nakoda an asset
- Signs of Safety training an asset (training will be provided)

- Must be available for on-call duties and availability to work weekends and holidays
