Looking for your COVID immunization record? There are options:

– The initial paperwork that was provided at the time of your 1st/2nd dose vaccination.

– Wallet-sized Stoney Health Services Proof of Vaccination. Call 403-881-3920. The card may take a couple of days to complete.

– Alberta Health Services Immunization Record available through MyHealth Records (starting September 19). Print a copy of your card-sized COVID-19 proof of vaccination or show it on your phone or tablet.

Albertans who need support can contact the MyHealth Records support line at 1-844-401-4016 from 7 a.m. to 7 p.m. Monday to Friday, and on weekends from 9 a.m. to 5 p.m.
